Renewing takes two steps: get a new recommendation from your physician, then renew your card in the state registry run by the Division of Cannabis Control. Renewing on time keeps you able to buy at medical prices without a gap.

Anyone with an Ohio registry account can renew, whether the card is still active or has already expired.1

Ohio residents with photo ID

Dispensaries check the government-issued photo ID tied to your registry account, so keep your address and ID details current.5

Adults 18+, or minors with a caregiver

Patients under 18 renew with a registered caregiver on their account.

A new CTR recommendation

Only a physician with a Certificate to Recommend from the State Medical Board can issue it, and the visit can be done by telehealth.78

When to renew

The registry emails you a reminder before your registration expires.1 Book your physician visit early so your new recommendation is ready when the renewal window opens.

Step-by-step renewal

  1. Get a new recommendation

    See your CTR physician by telehealth or in person. After your evaluation, the physician enters your new recommendation in the registry.1

  2. Sign in to the registry

    Go to the Ohio Medical Marijuana Registry and log in with the email and password you used when you first activated your account. Forgot your password? Use the registry’s login help.1

  3. Click RENEW CARD

    The red button appears once you’re inside the 30-day window, or once your new recommendation is issued if your card already expired.1

  4. Check your details

    Confirm your name, address and contact information, and update anything that has changed since last year.

  5. Download your new card

    Your updated card appears on the left side of your profile with a new expiration date. Select Download to save or print it.1

If both your card and recommendation have expired

You need a new recommendation first. The RENEW CARD button won’t show up in your account until your physician issues it.1

Renewing a caregiver card

Caregivers renew separately, in their own registry profile.2

Timing

Caregivers can renew up to 90 days after their registration expires, but the Division strongly encourages renewing in the same month the card expires.2

Approval before buying

After a caregiver clicks RENEW CARD, the program must approve the renewal before the caregiver can purchase for the patient again.2

Frequently asked questions

How much does it cost to renew?

The state patient fee was cut from $50 to one cent on March 4, 2024, and the Division has moved to eliminate it entirely.4 Your physician visit is a separate cost; with Ohio 420 Doctors, renewal is $149.

Can I renew early?

You can see your physician any time, but the registry only lets you renew your card within 30 days before your registration expires.1

My card already expired. Can I still renew?

Yes. Get a new recommendation from a CTR physician, then sign in and click RENEW CARD. You won’t need to start a new registration.1

Can I buy at a dispensary while my card is expired?

Not as a medical patient. Adults 21 and older can still buy adult-use cannabis, but they pay the 10% adult-use excise tax and lose medical benefits such as the four-day bulk purchase.6

Do I need to see the same doctor as last year?

No. Any Ohio physician with a Certificate to Recommend can issue your new recommendation.7

Step‑By‑Step Ohio Medical Marijuana Card Renewal

  1. Step 1 – Get a New Physician Recommendation: Schedule a telehealth or in‑person visit with an Ohio‑licensed medical marijuana doctor. After your evaluation, the doctor updates your recommendation in the Ohio Medical Marijuana Registry.
  2. Step 2 – Log In to the Ohio Medical Marijuana Registry: Within 30 days of your registration expiring (or after your new recommendation is issued), go to www.ohiomedicalmarijuanaregistry.com and log in with the same email and password you used when you first activated your card.
  3. Click “RENEW CARD” in Your Profile: Once logged in, open your Registry Details section, go to the Card tab, and click the red “RENEW CARD” button next to your current registration.
  4. Review and Confirm Your Information: Verify that your name, address, and contact details are correct and update anything that has changed since your last renewal.
  5. Submit Your Renewal (No State Fee): Submit the renewal; there is currently no state card renewal fee, but your physician may charge $45–$150 for the recommendation visit, depending on the clinic.

Renewing a Caregiver Medical Marijuana Card in Ohio

  • Caregiver registrations do not require a physician’s recommendation, but must remain linked to an active patient registration.
